Connecting enrollees with primary medical care providers

 On May 22, 2014, the Board of Trustees of the Metro Health Foundation awarded a grant of $20,000 to The Information Center to “establish a collaboration of organizations in Out-Wayne County and to coordinate Healthy Michigan Plan outreach, education and enrollment activities with the ultimate goal of connecting enrollees with a primary medical home.” The Healthy Michigan Plan, Medicaid expansion, provides a great opportunity for the uninsured in the community to get medical insurance coverage and access to quality health care.

The Information Center will collaborate with enrolling Healthy Michigan agents to coordinate outreach, enrollment activities among the various agents and assist agents in connecting enrollees with primary medical care providers. In July, The Information Center will be arranging a gathering of the various enrolling agents (navigation partners) serving Out-Wayne County to develop a model to effectively reach all eligible Healthy Michigan applicants.

President/CEO of The Information Center, Edward D’Angelo said, “It’s an honor for The Information Center to be entrusted with this responsibility to bring together organizations serving Out-Wayne County residents to partner and help those without health insurance obtain health insurance coverage.  It is important to families with people who do not currently have health insurance. It is important to health care providers who need to be reimbursed for the services provided to these people in order to continue to provide services, and it is important to the State of Michigan to access the Federal funds available. ”
