The Information Center celebrates National Nurses Week

The Information Center, Taylor nonprofit, celebrates National Nurses Week and the professional work of TIC Care Management nursing staff.  Recently, The Information Center expanded its Nursing Facility Transitions Program (NFT) to include: Nursing Facility Transition Specialist, Kelley Copp, R.N., (Developer/Director of The Info Center-NFT Program), Supports Coordinators, Diana Elledge, R.N., Myra Diggs, S.W. and Marsha Burks, S.W.

The Nursing Facility Transition Program is designed to assist senior citizens and disabled adults who are currently residing in a nursing facility and want to transition back into the community. NFT is funded in part by Medicaid funds provided by the Michigan Department of Health and Human Services. (MDHHS)

Nursing Facility Transition Specialist, Kelley Copp, R.N., “It is an honor to return to The Information Center to continue the important work of the Nursing Facility Transitions Program. Our supports coordinators do their very best to assure that our clients are the center and main focus of all of our efforts. Our NFT team consists of a compassionate group of professional nurses and social workers who truly care about their clients and the community they serve.”

Recent Michigan Department of Health and Human Services reports ranked The Information Center number two in the state, scoring second best in their Quality Assurance Review. This ranking recognizes The Information Center for exceeding minimum standards and as a result TIC received 101 percent of their quality withholding. Quality Assurance in Heath Care is a system for evaluating performance, as in the delivery of services or the quality of products provided to consumers, customers, or patients. The Information Center also ranked highest in Acuity in the Wayne County region. This ranking recognizes The Information Center Care Management staff as capable of providing high quality services for high need clients.

 

Additional Programs Provided by TIC:

The MI Choice Medicaid Waiver program is a comprehensive, community–based program designed for Medicaid-eligible adults who need nursing home level of care. The goal is to provide individuals with a choice of remaining and living independently in the community with support services, instead of going to a nursing home.

Trusted Care Solutions is a private pay care management program that empowers and educates at risk adults allowing them to make informed choice that will secure their safety and independence. The goal of this program is to provide education, resources, and services that will prevent premature nursing home placement.
