Nursing Home Transitions Initiative

NEW OPTIONS are available for Medicaid-eligible senior citizens and handicapped individuals that have been residing in a Nursing facility for at least 6 months and want to transition into the community. Individuals may transition either to a private residence or to an assisted living facility depending on the specific needs of the individual. Call our office at 734.282.7171 to learn more about the Nursing Facility Transition Initiative (NFTI) - Our Nurses and Social Workers think it’s Nifty!
