Nursing Home Transitions Initiative

NEW OPTIONS are available for Medicaid-eligible senior citizens and handicapped individuals that have been residing in a Nursing facility for at least 6 months and want to transition into the community. Individuals may transition either to a private residence or to an assisted living facility depending on the specific needs of the individual.
