Nursing Facility Transition
The Nursing Facility Transition Services Program is designed to help seniors and disabled adults who currently live in nursing facilities transition back into the community. Based on specific needs, individuals may choose to transition into either a private residence or an assisted living facility.
We have resources to help with everything from security deposits to cleaning to purchasing household goods. To be eligible for this program, you must be a:
• Current Medicaid Recipient
• Resident of one of the 34 communities in Southern or Western Wayne County
• Resident of a nursing home for a minimum of six months -OR- Person in imminent danger of nursing home placement and wants to remain in the community
Funding for this program is provided through the Michigan Department of Community Health.
