Private Pay Case Management Services
Our care managers are social workers and registered nurses with many years of experience planning and arranging a wide range of services that are provided in the home. We can connect you to services you may not have known about such as meals on wheels, in home doctors, veterans programs, socialization options, homemaking and personal care assistance, medication monitoring, etc.
For a reasonable fee, you will receive a comprehensive needs assessment and a presentation of your person centered options. We support and honor your individual choices and create a plan that focuses on how you want to live. You make decisions as to how you would like to proceed with your plan. You can direct your care, choose your care providers and remain in control of your life and finances with the knowledge of available services and supports.
