Care Management Services

"My dad is 87 years old. He just had a stroke and is currently in a nursing home for rehabilitation. He's almost completed his physical therapy and mom wants him to come back home. He needs more help than she can give, and I work full-time. I want to help them, but I don't know what services they are going to need or where to get them..."

Care Management at The Information CenterHelping elders plan for in-home care is what Care Management Services are all about. These services help the frail elderly who may be at risk of nursing home placement. The goal is to help locate and arrange for care at home, taking into account personal needs.

What does Care Management Include?

  • A comprehensive in-home assessment of care needs, conducted by a nurse and social work team;

  • Personalized planning of care options to meet the needs of the elder being served;

  • Arranging services based on the elder and their family's preferences;

  • Follow-up visits in the elder's home at least every 90 days, conducted by a nurse and social worker to adjust the care plan as needed;

  • Purchasing services (on a limited basis) for those who do not have other resources.

Who is eligible?
You must be 60 years of age or older and you must have a medical need that requires assistance. If you can answer "yes" to any of the following questions, you may qualify for services.

  • Are you unable to meet all of your daily care needs (such as meals, bathing, keeping up the housework)?

  • Have you had repeat visits to the hospital or emergency room?

  • Are your family, friend, or other helpers unable to assist you to the extent you need?

  • Are you thinking about moving into assisted living housing or a nursing home to get the care you need?

  • Are you living in a nursing home and want to know if services can be arranged at home to meet your needs?

If you answered yes to any of these questions, help is a phone call away. When you call, we will ask you some questions to determine if you are eligible for Care Management. If you are not eligible, we will help you sort out other possible solutions.

Who pays for services?
Participants will be asked to contribute toward direct care services that they approve (such as personal care, meals, or homemaking). Some services may be covered by your health insurance or by social services programs. Care Management services are donation based. No services will be arranged without your approval.
