Mrs. H referred herself to the Medicaid Waiver program and was determined to be eligible at her initial home assessment for the program one-week later. Mrs. H was 76 years of age with severe pulmonary fibrosis. She lived alone and had two sons who provided for some of her care needs. In addition, she was receiving hospice services at the time of referral. While hospice was able to provide much emotional support, her medical and personal care needs were more than they could provide.
After enrolling in the Medicaid Waiver program, Mrs. H received additional personal care services three days a week. She also began seeing a nurse who came to the home on a weekly basis. Additional services arranged for her included in-home podiatry services, a homemaker to assist with meal preparation three times a week, and snow removal as needed.
After the first reassessment, Mrs. H mentioned that her electrical service in the home was inadequate and could not support her need for continuous oxygen. She was using two concentrators at the time and was constantly blowing fuses. Electrical upgrades are not a regularly provided service through the Waiver program, yet in this case was essential to meeting her needs at home.
The Care Managers obtained a total of three bids from electrical contractors. A total upgrade of her electrical system was accomplished using funds granted through the Taylor Community Development Block Grant dollars allocated to The Information Center. Using this funding source and combining it with the Waiver services resulted in a solution to her problems. At the last reassessment visit, Mrs. H reported that she was able to run her oxygen as needed.
